i think you might find one in Kanyon, nt sure of the name , could be MM shirts, they scan you so that the shirt is a near to perfect fit, i heard that they are the best but also expensive.

What is the range of prices on custom-made shirts? I'm esp. interested in finding someone to do reasonably priced double-cuffed shirts.
